Booking Forms

We've added the ability for you to customize and create multiple booking forms (more details on bookings here), similar to the customization of our sign-up forms. This includes the ability to add custom fields to the form, as well as capture commonly asked for fields such as date of birth and parent name. In addition, users can now assign a document to be signed during the booking, a frequently feature. 

Creating a Booking Form

To create a booking form, navigate to settings -> bookings -> booking forms. Here you can either edit a booking form via the pencil icon, obtain a QR code, or add a form.

You'll have the option to select where each form is utilized under the Form Settings. Your choices include:

  • Gymdesk website (if you use the Gymdesk website builder)
  • Manager account (staff-led bookings under Gym-Schedule)
  • Booking widget (if you've embedded the booking form to an external site)
  • Member portal

Form Customization

Additionally, you'll find the ability to customize the following:

  • Form name
  • Name label (for example if you prefer the name capture field to be "student name" to be clear it's for the student and not the parent)
  • Fields (select the information you'd like to capture for a booking & what's required)
  • Document signing (if you want any trialers that booked a class to sign a waiver ahead of time)

 

Submission & Tracking

You'll also notice the option to add a post-submission message with any additional context you'd like to provide, as well as the ability to redirect to a link (can be used for tracking purposes) or add a conversion snippet (for tracking submissions via tools like Google Ads)

 

Automations

The bookings forms are also selectable for triggering specific automations (more details on automations here), allowing creating automations that target different booking forms.
