[Members](https://docs.gymdesk.com/ja-jp/help/members?hsLang=ja-jp) # メンバー登録フォームの管理と作成 サインアップフォームを使うことで、新規メンバーをオンライン上で、あるいはフロントデスクに設置したデバイスを通してデジタルで登録することができます。各 Gymdesk アカウントにはデフォルトのサインアップフォームが含まれており、これをカスタマイズしたり、プログラム別・年齢層別・プロモーション用などに複製したりできます。 また、特定の対象者やメンバーシップタイプ向けに追加のサインアップフォームを作成することも可能です。フォームごとに、必要に応じて独自の入力項目、メンバーシップ、提出書類などを設定できます。 **サインアップフォームへのアクセス方法** メインメニューの **Settings（設定）**に移動 **Members（メンバー）**→ **Sign Up Forms（サインアップフォーム）**を選択 ![Picture2-Nov-19-2025-03-38-43-8039-AM](https://docs.gymdesk.com/hs-fs/hubfs/Picture2-Nov-19-2025-03-38-43-8039-AM.png?width=670&height=593&name=Picture2-Nov-19-2025-03-38-43-8039-AM.png) **サインアップフォームのトグル設定** - **Manually Confirm Website Sign-ups（ウェブ経由のサインアップを手動承認）** – 有効にすると、オンラインから登録された新規メンバーは、メンバーリストに追加される前に承認が必要になります。無効の場合、サインアップ完了と同時に自動で追加されます。 - **Get Notified When a New Member Signs Up（新規メンバー登録時に通知を受け取る）** – 有効にすると、誰かがサインアップを完了するたびにメール通知が届きます。無効の場合、通知は送信されません。 **サインアップフォームの更新・削除** 既存のフォームを更新するには **✎ Edit（編集）** アイコンをクリックします。削除する場合は赤いゴミ箱アイコンをクリックします。削除前には最終確認メッセージが表示され、承認すると削除が確定します。 **サインアップフォームの表示と共有** フォーム名をクリックすると、公開版が新しいタブで開きます。右クリックすれば、フォームへの直接リンクをすぐにコピーできます。また、**QRコードアイコン** を選択すると、ダウンロード可能な大きなQRコードが表示され、フロントデスクでの掲示や印刷にも便利です。 ![Picture3-Nov-19-2025-03-39-57-3501-AM](https://docs.gymdesk.com/hs-fs/hubfs/Picture3-Nov-19-2025-03-39-57-3501-AM.png?width=670&height=413&name=Picture3-Nov-19-2025-03-39-57-3501-AM.png) **新しいサインアップフォームの作成** **Add Form（フォームを追加）**を選択すると、新しいサインアップフォームを作成できます。そこから、フォームの利用場所、表示内容、必須情報など、ワークフローに合わせてあらゆる項目をカスタマイズ可能です。フォームを調整することで、訪問者への見え方や、登録時に収集する情報を柔軟にコントロールできます。 **フォーム詳細** フォーム詳細セクションでは、サインアップフォームの基本情報を設定します。内部用の名称や利用場所を指定し、必要に応じてサイト上に表示するタイトルも追加できます。 ![Picture4-Nov-19-2025-03-41-01-7689-AM](https://docs.gymdesk.com/hs-fs/hubfs/Picture4-Nov-19-2025-03-41-01-7689-AM.png?width=670&height=233&name=Picture4-Nov-19-2025-03-41-01-7689-AM.png) - **Form Name（フォーム名）**： 　後で目的がすぐ分かるよう、分かりやすく具体的な名前を付けてください。これは内部管理用であり、フォーム記入者には表示されません。 - **Default Form For（フォームの使用場所）**： 　フォームを使用する場面を選択します（例：マネージャーアカウント、フロントデスク、ウェブサイトサインアップ、メール招待 など）。 - **Title Above Form（フォーム上部タイトル）**： 　フォーム上部にタイトルを表示します（ウェブサイト用サインアップフォームの場合のみ）。 **会員情報** 会員情報セクションでは、サインアップ時に表示・入力させたい個人情報項目を選択できます。「目のアイコン」で表示／非表示を切り替え、「アスタリスク」で必須項目に設定できます。「Allow Type Selection（タイプ選択を許可）」をオンにすると、サインアップ時に本人が会員タイプを選べます。オフの場合はデフォルトのタイプが自動適用されます。 **会員タイプは次の 2 種類です：** • **MEMBER（メンバー）**：すべての会員プランに**フルアクセス**できる個人 　　　　または • **VISITOR（ビジター）**：**トライアル会員プランにのみ限定アクセス**できる個人\] ![Picture5-Nov-19-2025-03-42-17-8756-AM](https://docs.gymdesk.com/hs-fs/hubfs/Picture5-Nov-19-2025-03-42-17-8756-AM.png?width=670&height=335&name=Picture5-Nov-19-2025-03-42-17-8756-AM.png) - **Check-in code（チェックインコード）**： 　サインアップ時に、会員自身がチェックインコードを設定できます。項目名横の丸を選択すると「必須」にでき、目のアイコンで非表示にもできます。 - **Custom fields（カスタム項目）**： 　会員設定で作成したカスタムフィールドがここに表示されます。このセクションから表示設定と必須設定を行うことができます。 **住所** **住所**セクションでは、会員の「住所・市区町村・都道府県・郵便番号・国」を入力できます。右側のトグルでこのセクション自体を**非表示**にすることも可能で、必須にしない場合は **Required（必須）**チェックを外して任意項目として扱えます。このセクションの項目は基本的に固定ですが、**State（州／都道府県）**のみ、必要に応じて非表示にできます。 ![Picture6-Nov-19-2025-03-43-00-0064-AM](https://docs.gymdesk.com/hs-fs/hubfs/Picture6-Nov-19-2025-03-43-00-0064-AM.png?width=670&height=176&name=Picture6-Nov-19-2025-03-43-00-0064-AM.png) **連絡先** **連絡先**セクションでは、*緊急連絡先、保護者、家族など*の追加の連絡情報を収集できます。セクション名は、フィールドタイトルをクリックして新しいラベルを入力することで、フォームの目的に合わせて変更できます。右側のトグルでこのセクションを**表示／非表示**にでき、必須にしたくない場合は **Required（必須）**のチェックを外せば任意項目として扱えます。 ![Picture7-Nov-19-2025-03-44-03-6609-AM](https://docs.gymdesk.com/hs-fs/hubfs/Picture7-Nov-19-2025-03-44-03-6609-AM.png?width=670&height=173&name=Picture7-Nov-19-2025-03-44-03-6609-AM.png) **活用例：**未成年会員を受け入れるジムで、保護者情報や緊急連絡先を記録する必要がある場合に便利です。 **写真** **写真**セクションでは、サインアップ時に会員の写真を撮影またはアップロードできます。右側の表示／非表示トグルでこのセクションを隠すことができ、登録時や本人確認のために写真が必須の場合は **Require Photo（写真必須）**にチェックを入れます。アップロードされた写真は、**メンバー一覧**と**各メンバープロフィール**に名前とともに表示されます。会員証を発行している場合は、会員の名前の横に写真が印刷されます。 ![Picture8-Nov-19-2025-03-45-03-7704-AM](https://docs.gymdesk.com/hs-fs/hubfs/Picture8-Nov-19-2025-03-45-03-7704-AM.png?width=670&height=156&name=Picture8-Nov-19-2025-03-45-03-7704-AM.png) **活用例：**Gymdesk の会員証機能を利用して、カードに会員写真を表示したい場合や、クラスのチェックインで写真による本人確認を行うジムに最適です。 **メンバーシップ** サインアップフォームの **Membership** セクションでは、登録プロセスの一部としてメンバーシップの選択を含めることができます。**メンバーシップ選択を必須にする**場合は、「Require Membership Selection」にチェックを入れます。右側の表示／非表示トグルを使えば、このセクション自体を非表示にすることもできます。 - **リダイレクトリンク**： 　スタンドアロンのウェブサイト用サインアップフォームの場合、送信後に任意のウェブサイトへ自動的にリダイレクトさせることができます。 - **サンキューメッセージ**： 　サインアップ完了後に表示される「完了メッセージ」をカスタマイズできます。 - **JavaScript コンバージョンスニペット**： 　Google Analytics、Google 広告、Facebook 広告などの外部トラッキングツールと接続するためのコードを設定できます。これはサインアップ完了後に起動し、トラッキングツールでコンバージョンとして記録できるようになります。 ![Picture15-3](https://docs.gymdesk.com/hs-fs/hubfs/Picture15-3.png?width=670&height=557&name=Picture15-3.png) **サインアップ招待** サインアップフォームを設定した後は、メンバー一覧から見込み客へ直接招待を送ることができます。メインメニューで「Members（メンバー）」に進み、黒い**「Invite（招待）」**ボタンをクリックします。見込み客の**名前、メールアドレス、電話番号**を入力し、件名とメッセージを調整して、適切なサインアップフォームを選択します。すでに希望するメンバーシップが分かっている場合は、事前に選択することもできます。 また、**リードプロフィールから直接「Invite to Sign-up（サインアップ招待）」**をクリックして招待を送ることもできます。 -
